I need help with setting up my modem in a hub

Hello everyone,

So like the title says, I’m trying to setup my modem to use this hub. [Modem Hub?] (I can take more pics if needed)

I’m not really sure what to do when it comes to this, because I’ve never used one before. Any help or insight would be amazing. The reason I took a picture of the sticker in there is because I thought it would be useful?
