2960X can't see fiberstore transceiver

I've got a WS-C2960X-48TS-L with a couple of fiberstore GLC-LH-SMD installed.

No signs of life from these guys according to show inventory, but I do see a small clue from show interfaces <interface> transceiver properties:

Empty hole:

Switch#show interfaces gi2/0/49 transceiver properties Diagnostic Monitoring is not implemented. Name : Gi2/0/49 Administrative Speed: auto Administrative Duplex: auto Administrative Auto-MDIX: on Administrative Power Inline: N/A Operational Speed: auto Operational Duplex: auto Operational Auto-MDIX: on Media Type: Not Present <--- This line

Interface with fiberstore transceiver:

Switch#show interfaces gi2/0/50 transceiver properties Diagnostic Monitoring is not implemented. Name : Gi2/0/50 Administrative Speed: auto Administrative Duplex: auto Administrative Auto-MDIX: on Administrative Power Inline: N/A Operational Speed: auto Operational Duplex: auto Operational Auto-MDIX: on Media Type: unknown <--- This line

If I move the transceiver over to an ASR, I can see it with show inventory:

NAME: "subslot 0/0 transceiver 6", DESCR: "GE LX" PID: GLC-LH-SMD , VID: A0 , SN: G1804197087

I tried enabling service unsupported-transceiver. This did not make any difference. When I removed the command, I got some log messages:

Oct 23 15:52:05.899: %PM-4-ERR_DISABLE: gbic-invalid error detected on Gi1/0/49, putting Gi1/0/49 in err-disable state Oct 23 15:52:05.899: %PM-4-ERR_DISABLE: gbic-invalid error detected on Gi1/0/50, putting Gi1/0/50 in err-disable state Oct 23 15:52:05.890: %PM-4-ERR_DISABLE: gbic-invalid error detected on Gi2/0/49, putting Gi2/0/49 in err-disable state Oct 23 15:52:05.890: %PM-4-ERR_DISABLE: gbic-invalid error detected on Gi2/0/50, putting Gi2/0/50 in err-disable state

This is a little weird. I only actually own two of these modules, but all four ports they've (ever) touched threw an error at the same time, right when I removed the command.

Thoughts? Is this just a case of getting what I paid for?

edit: I'm starting to think the issue might be CSCul88801
