RJ11 crimped to Cat5/5e/6, any limitations?

I'm working with a modem which only accepts an RJ11 plug, but I need the robustness of a Cat5/5e/6 cable. I know the RJ11 can for sure be crimped to a Cat5 cable, but has anyone had any experience with Cat5e or Cat6? Are there any standards for dimensions (like cable diameter) that RJ11 accepts?
