Unreliable FS:COM Branded 40G Fibre Module

Hi,

We have 2x fibre links between an ICX7450 & ICX7750, both using a 40G module in each switch, with a LAG+LACP between the pair of links. One link works fine, and the other has gone offline twice in 2 months. Last time I spent a day troubleshooting and could only get the link up again by deleting the LAG, removing the modules, and installing again from scratch. Now, I cannot get the link up again at all.

I had a second opinion from a remote networking expert we have used in the past, who confirmed nothing wrong with the config on either switch. He also could not help explain why the link failed. We both suspect it is one of the 40G modules at fault. They are not branded, only having a sticker from the supplier FS.COM (40GE-LR4 10km (QSFP+ LC). We suspect a module, because show media command shows one of the module bays as empty, on one side of the broken link. What is confusing, is the web interface shows port stats, albeit status down. We are using Fiber Optic Patch Cables, Single Mode, OS2, Duplex cabling.

Has anyone experienced something frustrating before with these kind of modules and an intermittent link?
